Has there been any occurrences? Do we know it can work outside of simulations?
How many systems have been installed?
 
it is a system that if the pilot becomes incapacitated, a passenger can push a button and the plane will automatically land at the nearest suitable airport.
...and every manufacturer gives it a different name, leading to confusion, even though it's a mostly-Garmin product.

I don't know of any saves yet, but I do know of at least one inadvertent activation. There was a guy who decided to show his friend what the LVL button did in his TBM, not realizing that if you hit LVL and then do not respond within two minutes, HomeSafe (Daher's name for it) is automatically activated... And he didn't know how to deactivate HomeSafe either.

Not sure about the others, but in the TBM, HomeSafe is activated in three ways: The big red button, not responding after Emergency Descent Mode following a cabin pressure alert, and not responding after hitting LVL. Once activated, you have to deactivate by hitting the A/P DISC *twice*, and then follow two checklists - One to get all the screens rebooted, and one to ensure you're not misconfigured, as the system can extend gear and flaps but does not actually move the knobs in doing so.

I believe there are videos of it being tested in a real aircraft in real world, but as others have said, I don't believe there has been a save yet.

It is honestly more of a feel good safety measure, even more so than a ballistic parachute. There are more scenarios of higher frequency of occurrence a parachute would aid in. The "Safe Return" is most likely to be used in a pilot incapacitation event which is pretty uncommon, although can be a concern of a frequent passenger in a single pilot aircraft, or in my case my wife.
