* easy for a reasonably-savvy senior citizen to use (asking for a friend!)
* can store multiple entries per day
* can add notes/comments to an entry
* does not report your medical data to some mega-corp or store it in some mega-corp's cloud
* no ads

I lost 35 pounds and wanted to get off of my blood pressure meds. I ordered this Blood Pressure Monitor from Amazon for $36. It records my blood pressure and gives me a score, red, yellow, or green, and records it on the app so I can share with my doctor. It doesn't meet your requirement of adding notes, but may help someone else though!

I also bought a smart scale for $24 that records to an app. Helped keep me motivated.

Sure it does. Heart monitors, bike computers, apps among others can be used to upload data to apple health. To me the biggest disadvantage to apple health is that you can't view data on a laptop. Only works on phone or ipad. This is driven by HIPPA concerns, not any technology issues.
