En-Route
Interesting thread. Since buying the thrifty 150 I'm flying with a new CFI. He's not a new CFI but, rather, to him I'm a new student. Even before taxi I reach behind his back and flip up the door lock which is at the rear of the door. That brings a question: why is there no door lock on the pilot's side? I was used to the C-172 whose both door locks were forward movement of the door handles.
What I'm suggesting is that it would very difficult for him to pull a truly open-door surprise, as has been the subject herein, because he would have to contort his body in some way to undo the door lock; and my peripheral vision would catch him in an otherwise unnecessary motion. And he's not questioned my nudging him forward of his seat-back when I reach over to check for "door locked", twice, before taxi and during runup(in the event he pulled some crap during taxi).
