So we are going to Belize for a few days. Despite my personal objection to the concept, we have to maintain ability to receive and place phone calls.

International roaming is wicked-expensive!

Assuming I have Internet available (supposed to be at the place we are staying), can I set up a SkypeIn number, or something equivalent, which would allow me to receive calls on my iPhone as VOIP calls?

If so, please describe; if not, suggested alternatives?

You should be able to do that if you have a wifi connection. International data rates are typicallly much higher than the US, and much higher than international voice roaming.

Options:

1) try and get a local SIM & unlocked phone. If you're using the DeathStar (ATT) network, I understand that unlocking an iPhone is not an option (but I'd still ask). That leaves you with the option of getting a cheap unlocked phone (voice only) or using another option. (Link to Telestial

2) I don't recall whether SkypeIn will send an email with attachment or not. I know Ooma will ('cause I use it...), GoogleVoice will (remembering that there's limited privacy there), and some of the other services will. Simply have folks leave a message & you call 'em right back on a cheap VoIP connection.

That would effectively prevent use of Skype, Ooma, or any other VoIP service (unless you use a VPN system to bypass the local controls). Forces you to another option.

3) Get an international roaming SIM card from someone like Telestial. No, not the cheapest solution, but in some countries incoming calls are free. It's still vastly cheaper than roaming with a US number. Again, you'll need an unlocked phone to take advantage of this.

Money-wise, it comes down to this: if you can't get your phone unlocked, buying an unlocked phone is not worth the money unless: 1) you're planning to travel a lot overseas, 2) you're going to be there long enough to pay for the phone in savings, or 3) you're going to make enough calls to make up the cost of the phone in savings w/a local SIM. Fortunately, older generation voice-only cellphones are pretty cheap to buy unlocked......
 
One other thing. If you do decide to get a local SIM card, it's cheaper to get it after arrival rather than through a company like Telestial.

The disadvantage of getting it locally is that you won't have the number before you leave (meaning that it'll be harder to set up forwarding/let the folks that need your number know in advance). What you CAN do in that case is to use something like Ooma, G-voice, or another system that will forward your calls & change the forwarding number on that service after you are activated locally. That does work well - I have a local SIM card for Italy and another for Spain that I use in that fashion.

Do you have to receive calls from everyone, or just a few select people?

I use Skype (Skype user to Skype user) 99% of the time when I'm overseas, unless it's an emergency. On the Iphone, you can turn airplane mode on, which turns all the services off, then turn wireless back on which will leave cell and data inactivated so you incur no roaming charges while your phone is on and accessing wireless.

I use Acrobits on an iPod Touch ... and the friendly folks at Acrobits have setup
an encrypted/SIP proxy that I use to tunnel around those that (attempt to) block
VoIP.

Call setup is a bit slower, but it works.
