Did any fail in flight as in head separation reulting in engine shutdown?

I had an ECI IO-550B4 jug fail at 4 hours TT on an A36Tn 3 years ago. The piston disintegrated on the takeoff roll...totally trashed the turbo , prop hub and engine.

Some days it's quite comforting to know I have the original cylinders on my plane.

Today is one of those days.

Take comfort in that it's not the piston that they're addressing, only the cylinder barrels.

What was the cause? Was it a manufacturing defect or an assembly error?
