Did any fail in flight as in head separation reulting in engine shutdown?

I had an ECI IO-550B4 jug fail at 4 hours TT on an A36Tn 3 years ago. The piston disintegrated on the takeoff roll...totally trashed the turbo , prop hub and engine.:mad2:
 
Some days it's quite comforting to know I have the original cylinders on my plane.

Today is one of those days. ;)

Take comfort in that it's not the piston that they're addressing, only the cylinder barrels.

What was the cause? Was it a manufacturing defect or an assembly error?
