I'm an instrument rated, private pilot, with a complex aircraft sign off. However, my medical is not current. My question is, without a current medical,
is it possible to get a high-performance sign off, without the medical being current?

Certainly. As long as the instructor has a medical (or basic med) and is otherwise qualified to be PIC.

Which he'd have to be anyway, since the OP couldn't act as PIC even if he had a medical.

That was my point. He couldn't receive any flight instruction (in an HP plane or otherwise) unless the instructor could be PIC.
