Your advice for an FAA approved neuropsychologist and Cogscreen Ae test

Hello everyone,
I'm an Israeli private pilot with American commercial license.
My medical certificate has been revoked by the FAA after I reported that I've started to take Cipralex.
I need to go through several steps in order to claim it back.
One of the steps is to submit a neuropsychologist report and passing Cogscreen-Ae test.
I'm interested in your recommendations for an FAA approved neuropsychologist and a place to take the cogscreen test, preferably both at the same location or area.
