Treating physician no longer practicing..

I just received my first correspondence from the FAA in reference to Anxiety I was treated for years ago. They want a copy of my records, which I have, and a current status, clinical exam, and report from my treating physician.

The problem is the physician that treated me is no longer in practice. How am I supposed to check this box off for the FAA?
 
Have your current physician provide it. He/she can note in the chart or provide a separate letter that details the current state of your anxiety diagnosis.
