It doesn't have the background but apparently, the web site was affecting how Adobe could create a PDF file.

But, here goes for the sake of preservation...

Kenny, it looked about the same as yours without the text header and footer.

The file shown was actually my first attempt. When I made a PDF from right-clicking, I got the same thing... no header or background. Then, I did a print to PDF and got what you see. The Adobe tool buttons were locked out in the IE tool bar.
