What would be the appropriate ICD code for seeing a therapist to support a mentally ill spouse and then to process one’s own feelings and ultimately decision to divorce? Current recommendation is F41.1Generalized Anxiety Disorder, however worry is not excessive, uncontrollable or irrational. Additionally, worry does not interfere with daily functioning. This does not seem appropriate. Thoughts or recommendations?

What would be the implications to medical?
Avoid taking medications if possible, keep in mind everything they take note of in sessions may wind up in hands of FAA for review. Be careful when talking about alcohol.
 
Why do you need a diagnosis code if there is no diagnosis? I'd avoid a diagnosis if possible. Pay cash and don't have them bill your insurance. I don't think marriage counseling is treated the same. See below copied from the instructions:

VISITS TO HEALTH PROFESSIONAL WITHIN LAST 3 YEARS – List all visits in the last 3 years to a physician, physician assistant, nurse practitioner, psychologist, clinical social worker, or substance abuse specialist for treatment, examination, or medical/mental evaluation. List visits for counseling only if related to a personal substance abuse or psychiatric condition.

If you weren't seen for treatment or evaluation, and your visits for counseling weren't related to a psychiatric condition or substance abuse, I would make absolutely sure you have to report it before doing so.
