Good day everyone,

I am on the back side of my 3 month recovery window from a mild MI and stent placement.
I am working on compiling my files to send to the FAA after a visit with my AME.

One of the FAA's requirements for a Second Class SI is an assessment from my Cardiologist which must include specific information. I am trying to find an example of a previously submitted assessment letter to try and ensure his assessment for me is complete and in a format acceptable to the FAA. A search through the threads here has turned up nothing. Any assistance would be appreciated. This is a new realm for me and I am trying to navigate it as best I can.

You won't find one because of HIPAA.
But the requirement is simple. He talks about:
Prognosis
Current limitations if any, and if none so stated
The quality of the management of your Cardiac Risk factors for progression of the coronary disease.

Another requirement for "return to service" for a 2nd or a first class is an pre-return to service Heart cath, looking for intimal fibroplasia. That is almost NEVER covered by health insurance and runs about $30K if they don't have to do ANYTHING. That's going to be your limiting factor (Unless you have Harvey Watt, the airline disability insuror).

Bruce, Thank you very much for the information, I figured this might be a difficult search but was hoping someone had a copy of theirs that they could redact their personal information out of and just leave the basics. My Cardiologist and his team were able to get creative with the second heart cath using the results of my GXT, I was fortunate that my insurance covered it.
