Pilots mistakenly left G-IV's flaps locked in place

Well, that's not what you stated.



They had SOPs and standardization, they just chose to ignore them.

Whenever someone looked at their operation or flying, they managed to put on a good show and stick to the script. There is no reason to believe that they couldn't have passed their twice a year ride with the FAA to maintain for example a 135 certificate.

So I take it you knew this operation personally?

Here's my take using previous experience: I've given part 135 evaluations (.293/.297/.299) and the ones that have been the weakest are the Part 91 corporate types moving over to Part 135. It's usually the same story, "yea, we're a 91 operation, but our manuals and procedures are all 135 based."

Someone who is consistent in non use of checklist and doesn't adhere to SOP's are easy to spot during a sim evaluation. If they have been getting 61.58 proficiency checks at the training center and now are subjected to a .293/.297 the game changes. The standards under 61 are much looser than 135 in the areas of checklist and call outs, as well as Procedures.
