Three years working in an ER as an EMT tells me that it ain't necessarily fake - doesn't LOOK fake, anyway! Frequently you can have something like this that doesn't bleed a whole lot, even when you take the thing out (which is itself a reason it might not be bleeding, with the scissors pluging the blood vessel). Heck, I saw a kid come in after a dirt bike get-off with pretty much all the skin on his forearm peeled away from the arm. No blood, to speak of!
