I know a couple of people who did this (the spreading of ashes). One of them got a face full. That can cause issues.
 
Sounds like it happened just after takeoff, within 1/2 mile of the runway. If so, unlikely it was during an attempt to spread ashes. Sad event no doubt.
 
Mrs. Steingar wanted me to do something like that with my mother in law’s cremanes. When I learned about some of the issues involved I decided not to. Airplanes are for the living. The down side is my mother in law is still a resident of my basement. Creepy.
 
Sad deal. I have written in my will that I am to be cremated and no one may posses any of my remains. The crematorium is to dispose of my remains.
 
Happens a fair amount, sadly. I'm aware of an older one where some lingering bone fragments from the cremains got jammed in the elevator... I was able to quickly find two more on the NTSB site in addition to the one Jeff posted:

It seems this sort of thing goes wrong more than it goes right, but most of the time it's just that the occupants/interior of the airplane get coated with Grandpa.
