I've been taking two medications for many, many years:
For the last several medicals, I have just listed them on the 8500-8 and checked "previously reported".
Last year I lost 45 pounds by modifying my diet. When I went to my yearly checkup with the cardiologist, based on my weight loss, she dropped the HCTZ completely and lowered my losartan dosage to 50mg daily.
Because of the different dosage, I assume I should list the Losartan 50 mg daily, and I should NOT check "previously reported". Will this trigger the need for a doctor's note, ekg, etc.?? If so, I'll go get those things. I just don't want to be in the AME's office and find out that he can't issue a medical on the spot.
I'm hoping Dr. Chien will chime in on this one.
