IME if an engine runs smoothly at low power but rough and weak at full throttle it's almost always fuel system related.

One way to look at it is with Otto flying and the pilot monitoring (diligently) both the autopilot and the pilot have to screw up to cause a problem. When only the pilot is doing all the work, one screw up is all it takes.

Plus there are plenty of places in NE where there's nothing close to "direct" when driving. The mountains there aren't a big obstacle for airplanes like the Rockies but they sure get in the way of the roads.
