Hello everyone,

I recently failed my MV and wanted to confirm if it will be internal or recorded on my PRD. I was informed by several colleagues in the company that it’s only internal, so it won’t appear on my PRD. I’m unsure if I need to disclose it on my application or if there's no need to.

Anyone in the training department of your airline you can ask?
 
Pretty sure airline apps asks for any training failures. It’s not really a big deal. Most guys have at least 1 failure. It’s not really a dealbreaker. You should report it. Worst case is you don’t and they find out and ask why you lied. Not worth it
 
Technically you don't "fail" an MV. AQP is a train to proficiency program. If any event during the MV is not SAT, then more training is given.

Now, after several attempts and the applicant is not making a SAT grade, the company can stop the training and evaluate whether to continue.

This. 3 airlines in, 2 with AQP and each record training differently. It will definitely show additional training and it may show an unsat event. Disclose on an application and interview. One is not a big deal. Two shouldn't be either unless it occurs again in a relatively short span of time (30 year career with 2 unsats is not the same as 2 unsats consecutively.

Also FWIW, this forum is more populated with GA pilots, not professional airline pilots....not that there's none here, but a question like you posed would fit in better on APC forums.
