Reporting Tinnitus to the AME

I have read a bit online, and it seems many flying pilots are diagnosed with tinnitus. How are we supposed to report it to the AME (my med cert. is up soon)?

I went to an ENT earlier this year, had an MRI to rule out other conditions. Should I try and get those records from the ENT and bring them with me, or is "I have tinnitus, I went to this Dr. on this date, the cause was loud concerts in my youth and noisy airplanes" ok ?

Don't volunteer any information.
 
AS long as you can hear, a bit of tinnitus is not an issue for an AME. But he'll be interested if there was dizziness associated. Bring a note from the ENT, though saying the is simple tinnitus and the MRI is negative.....

It will be a yawn.
