New to the site all, ALOHA from Savannah! I'm an I/A here at Gulfstream and I recently mailed my Bi-Annual I/A renewal packet to Atlanta w/ all my required paperwork, that had to be post marked by 31 March. Long story short, my packet was shredded by the post office, all the paperwork was lost and they received an empty envelope. I had an old phone number listed w/ the FSDO and instead of following up w/ sending me notice in the mail, the FSDO Inspector just wrote me off...

I of coarse thought they were back-logged, but I finally emailed and got an answer from the Inspector in regards to my packet...

He sent me the instructions to re-submit and also sent me a Pilot's BOI-I/A version as seen in the attachments, and he instructed me to fill it out and submit it w/ my renewal packet.

Question is, what in the world is this form and what is it's purpose? Should I assume I am under investigation? I know my packet did not make it by deadline, but there were justifiable circumstances....

You are not under investigation, but they now must inform you that the information you submit can be used if they do.

Basically just don't lie and you will be ok regarding the IA forms.
 
Wouldn't think of strolling down that road, but was COMPLETELY taken back when he mandated it being submitted along w/ my 2nd attempt...

Appreciate your reply Duncan! I figured one of you all knowing Pilots would have the answer!!!
 
Me too, it's something new this year and has nothing to do with your submission being the second attempt. If the first had made it through they still would have told you to send the additional form.
