You saw that story, too? Propulsion quits, no brakes! How would you like to drive up to the marina to go sailing, and your boat is nowhere to be found (unless you dive on it).

I suppose its better than going to the marina the night before and sleeping on your sailboat.
 
It was caused by a missing cotter pin.

A missing 3 cm cotter pin set off a chain of events that led to the grounding of a ferry at Horseshoe Bay in West Vancouver last week.

BC Ferries says a control arm connecting the engine speed control device to the engine fuel rack disconnected when a nut came off the attachment bolt.

The nut came off because an inexpensive cotter pin that is normally in place to hold it in place was missing.

It is always so amazing that a part that cost maybe a dollar caused a wreck of many millions. A missing cotter pin. Wow I will bring spares if I get on the ferry. The Chief Engineer may need an extra one.

It's happened with airplanes, too. I know of one that lost one end of the throttle cable for that very reason...
