    It can and does happen. It happened on a commercial flight I was on in January. The windshield cracked and the cabin depressurized. The oxygen masks dropping down and following emergency descent made for some excitement in the cabin.

    Most passengers, nothing. My significant other flies often enough with me that she’ll listen in on the radio for traffic advisories and help me spot traffic (as long as she isn’t sleeping, or taking pictures). I may also have a passenger change out my headset batteries when they go flat.

    The R44 has fairly benign autorotation characteristics. In this case however, based on the video, the sink rate seems high and the flare poorly timed. Maybe RRPM got too low. I’d be curious to see the preliminary report.
