    Correct. It indicates a revision to the procedure. After they get to 9, they go back to 1. IE, the JFK9 departure which became the JFK1 departure a few years ago.

    It is very situational. When I interview pilots, some have better ways of talking about previous checkride failures than others. Good interview prep can significantly help. If the applicant answers honestly and describes how he/she learned from the event, it is usually a non-issue.

    Whoops...I didn't realize you were talking about minimum altitude on the STAR for each segment. I thought you were talking about the expected altitudes. That notation is only for crossing restrictions. The FL190 and 11,000 ft notations on this arrival are MEAs.

    Correct. Unless it's an RNAV arrival and ATC clears you to, "descend via the XYZ arrival." We get that all the time going into Las Vegas (KEPEC3 ARR).

    It depends where the unit is mounted. If it is behind the "pilot's field of view" which the FAA has established, then I believe a separate annunciator is required. AC 20-138 spells it out... +/- 15 percent up to +/- 35 percent horizontal.

    It's both. You have to be able to meet the climb gradient while following the DP. In the case of HND RWY 17L/R, if you can't do that, you can climb in visual conditions as long as the weather is 2100-3 or better.
