Did you ask the controller for vectors or did the controller initiate vectors a little later than you were used to? As to your other questions, barring an approach clearance or vectors, I would expect you to fly PSB direct KUNV at your present assigned altitude and enter a standard holding...

Yes, the D-side position allows for all the FP elements except SAR info (acft color, SOB, FOB, etc). I used to ask for those three on a popup just to get it on the recording.

ERAM should know every published fix in the nation and many outside.

All enroute facilities have ERAM, and all flight plan processing is done by ERAM and forwarded to terminal facilities. If a terminal controller inputs a flight plan, he/she is interacting with ERAM, which will in turn transmit the necessary data to the STARS (terminal) computer system.

Automation doesn't care. File direct. If there is an adapted route required, ERAM will apply it and you will get an amended route.

I can't recall ever sending someone to FSS to file a FP. In defense of the terminal guys, their equipment was not laid out as conducive as ours for inputting FP information (it could be different now).
